| Licensure: | Licensed as a social worker in ND (LSW). Must have current valid drivers license. | |
| Experience: | Preference will be given to individuals with one year of experience in Hospice, Home Care, Hospital, Public Health or other health care setting. Experience working with families, groups and individuals is desired. | |

| Other: | Responsible for the overall implementation, provision, and coordination of social services offered to patients in the Home Care/Hospice programs in Garrison, North Dakota. | |
