    Purpose
    To present the latest information on health-related, end-of-life and Alzheimer’s issues affecting senior adults. General session and workshop speakers will present information, identify challenges and suggest appropriate responses to health-related and end-of-life concerns and Alzheimer’s disease.

    Audience
    Primarily for professionals and other staff who provide health-related and supportive services to senior adults. Family members and seniors may also benefit from attending.

    When
    Thursday, October 16 (all day, 7:30 AM to 5:00 PM) and Friday, October 17, 2008 (7:30 AM to 12:00 Noon only). There is a special session on Thursday, October 16 evening from 7-9 PM.

    Best Western Ramkota Hotel, 800 S. 3rd Street, Bismarck, N.D.

    Accommodations
    Participants are responsible for making their own reservations. A block of rooms has been set aside at the Ramkota Hotel for October 15 and 16. The Ramkota Hotel phone number is (701) 258-7700 or 1-800-528-1234. When making reservations, please indicate you are attending this conference.

    Objectives
    At the conclusion of this conference, participants will be able to:

    • Discuss information on healthcare, end-of-life and Alzheimer’s-related needs of senior adults.
    • Identify health-related challenges and appropriate responses.
    • Describe state and national perspectives on service delivery issues.
    • Identify methods for professional caregivers to better manage stress and the work environment and deal with difficult people.

    Continuing Education Credits
    St. Alexius Medical Center is an approved provider of continuing nursing education by CNE-Net, the education division of the North Dakota Nursing Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Application has been made for 9.25 contact hours. *

    Social workers will receive 9.25 CEUs. *

    Certificates of attendance for 9.25 contact hours will be issued to other participants upon request.

    ** An additional 2.0 contact hours/CEUs will be awarded if attending the Thursday evening “Family Caregiver’s Workshop.”

    Payment Policy:

    Registration Form
    Make copies for each person planning to attend the conference. A registration form must be filled out completely for each person. Registration cannot be shared by individuals. A CHECK MUST ACCOMPANY REGISTRATION FORM. NO VOUCHERS WILL BE ACCEPTED. Without payment you will not be considered registered. Registration deadline (postmark) is Friday, October 3, 2008.

    Registration By Website:
    If you are registering by using this website, credit card payment must accompany all registrations.

    Fee:
    The registration fee is $150 for both days. This includes materials, lunch and health breaks. A check must accompany registration. Registration cannot be shared by individuals.

    Late Registration:
    Pre-registration is required. If pre-registration is postmarked after Friday, October 3, 2008, there will be a $25 additional charge or $150 registration fee. On-site registration fee is $150, space permitting.

    Cancellation:
    The registration fee less $20 will be refunded if you choose to cancel before October 3. Conference fee may be transferred to another person without penalty. No refunds will be given if requested on or after October 3.

    Program Changes:
    St. Alexius Medical Center reserves the right to make program changes or to cancel the conference. In the event of cancellation, only those who pre-register will be notified and their fees will be refunded in full.
