Care Coordination services are provided to patients and/or families on
medical, surgical, telemetry and intensive care unit. Care Coordinators
work with social workers, Quality Management/Utilization Review Nurses,
physicians and other health care providers in a team concept to provide
clinical care coordination, resource management, transition planning and
utilization management. The predominate patient population age groups
served are adult and geriatric. Occasionally an adolescent or child may be
in ICU or on the Trauma service.
